Titer showing immunity. Hepatitis B Titer showing immunity.

MMR (Measles, Mumps, Rubella) OR Titers showing immunity (for each) 2 vaccinations

Varicella (Chickenpox) OR Titer showing immunity. 2 vaccinations.

Varicella (Chickenpox) NOT History of Chickenpox.

Tdap (not DPT, Dtap, or Td) 1 vaccination. If Tdap is ≥10 years: OR + Old Tdap Booster Tdap

Tuberculosis Screening (TB) Initial Screening 2 Skin Tests (PPDs) Within Year.* Chest X-Ray Results. Lab report. OR OR *Prevents false negative.

Free TB Skin Testing Events Do NOT get a skin test if you tested positive in the past! Schedule on website: www.unthsc.edu/immunizations

Medical History and Physical Examination UNTHSC form is recommended, not required. Documentation submitted must have similar information. 4 pages
