CRISP Core Services POINT OF CARE: Unified Landing Page & In-context Information Search for your patients’ prior hospital records (e.g., labs, radiology reports, etc.) Monitor the prescribing and dispensing of PDMP drugs Determine other members of your patient’s care team Be alerted to important conditions or treatment information CARE COORDINATION: Encounter Notification Service (ENS) Be notified when your patient is hospitalized in any regional hospital Receive special notification about ED visits that are potential readmissions Know when your MCO member is in the ED POPULATION HEALTH: CRISP Reporting Services (CRS) Use Case Mix data and Medicare claims data to: Identify patients who could benefit from services Measure performance of initiatives for QI and program reporting Coordinate with peers on behalf of patients who see multiple providers PUBLIC HEALTH SUPPORT: Partnerships with Maryland MDH, District of Columbia DHCF, and West Virginia through the WVHIN PROGRAM ADMINISTRATION: Technical and administrative support for Care Redesign Programs

Prescription Drug Monitoring Program Reminder: The query mandate for prescribers of controlled substances goes live on July 1. Also note that 42 CFR Part II data will not be included (no methadone programs) PDMP data is available in the Unified Landing Page Features include sorting and multiple patient selection

Encounter Notification Service (ENS)

Hospital Registration How does ENS work? A patient goes to the hospital Hospital Registration At registration the hospital asks the patient for basic information (name, DOB, etc.) and the reason for the visit. The registrar enters that information into an Electronic Medical Record. When the registrar has completed entering the information, and pushes ‘save’, a copy of that information is immediately sent to CRISP A provider who has submitted a patient panel to CRISP that includes this patient receives a real-time or batch notification that the patient has been to the hospital. 12

CRISP Reporting Services

CRISP Reporting Services CRISP Reporting Services (CRS) provides secure access to Maryland health care data and related analytics tools to assist health care organization in improving patient care throughout the state. CRS products are approved by the Reporting and Analytics Committee, a subcommittee of the CRISP Board, and adhere to best practices in privacy and security. 11/16/2018

What reports are available for ambulatory providers? CRISP is able to provide Panel-based reports to ambulatory organizations There are 2 reports that these organizations may access: Panels for Practices Pre/Post Analysis 11/16/2018

Users may filter data by Hospital, Payer, and Age Group Panels for Practices The Panels for Practices reports allow users to see various data sets for their patient population: Panel Utilization: Ability to see if patients on your panel are on panels for other organizations Hospital Utilization: Ability to see hospital (IP, ED) data for patients on your panel. This includes readmission rates Unique Patient Utilization: Shows data around how many visits a particular patient had, along with total charges associated with those visits Chronic Conditions: Allows users to see patients who have one or more chronic conditions Bedded Care by Service Line: Shows patients who have had a bedded hospital visit, by service line Users may filter data by Hospital, Payer, and Age Group 11/16/2018

CRISP Pre/Post Analysis CRISP developed the Pre/Post Analysis to help users who have implemented care programs understand hospital utilization before and after a specific enrollment or intervention date for each patient. The report also provides a self-service feature for a user to load a panel and generate results. Users can load multiple panels to see pre/post analysis for different programs 11/16/2018

Pre/Post Report Features Summary: Provides various metrics for the submitted panel of patients based on 1, 3, 6, and 12 months pre-post reporting periods. Panel Analysis: Allows users to see charges, # visits and # of patients by hospital, with a pre-post comparison Breakdown of Charges: Shows charges billed to specific service lines before and after the enrollment/intervention 11/16/2018
