Contributing factors to the high cost of healthcare in the U.S. Focus on sick care instead of well care Lack of Universal Health coverage Market factors Low levels of regulation and high levels of lobbying in healthcare Use of new technologies for competitive advantage and a shift to outpatient care Lack of Health IT and coordination of care Defensive medicine High rates of obesity and chronic disease High cost of aging and end of life care Lack of patient accountability Slide 2C Example

Research Objective 1 Describe research objective Slide 3 Template

HOSPITAL UNIT OF CARE (HUC) Definition: Set of resources required for delivery of one general medical-acute care inpatient day which includes the needed healthcare staff, ancillary and support services and facilities to deliver the required (acceptable quality) continuum of care. What is not included: Outpatient care and services, specialty or intensive care, surgeries, prescribed services, prescribed diagnostics, enhanced facilities, patient complexity etc. Our approach: To define an equivalency parameter for each of the additional care/services that the hospital provides. This allows us to roll-up all the reported hospital direct patient care activities into a unified output measure. Specifically we focus on a federally standardized list of activities. Six activity components are modeled. Example: 1 Intensive Care Patient Day = 2.3 HUC Which means an ICU day requires 2.3 additional units of resources compared to a general med-surg day Slide 3 Example

Research Objective 1 Results Describe results – Pictures, graphs, Analytics Slide 4 Template

Research Objective 1 Results Describe results – Pictures, graphs, Analytics The results show that both the BRE and NRE are significantly different from the HRE. Using the BRE or NRE to evaluate hospital productivity is clearly not a precise method and could lead to erroneous conclusions and strategies. The HRE can be used as the basis to evaluate productivity of all hospitals at the national level. Average HRE = $534 for the Honor Roll hospitals, providing an initial benchmark for analysis. Slide 4 Template

HUC ACTIVITY TO HOSPITAL OPERATING COST - Correlation To validate the proposition that HUC is a more reliable indicator of hospital output relative to APD, a regression model of operating costs against (i) HUC and (ii) APD was studied. Both a linear and power models. Data set included 203 US Hospitals. Operating Cost vs. Annual HUC (181 Hospitals) Linear R2 = 0.80 Power R2 = 0.82 Operating Cost vs. Annual APD (169 Hospitals) Linear R2 = 0.191 Power R2 = 0.141 Slide 4 Example

HRE STATE LEVEL ANALYSIS - CAUSAL FACTORS STUDY (NJ) Hypothesis Test #1: HRE is a function of hospital size Group 1(Small): < 250 Beds (N = 23) Group 2(Large): > 250 Beds (N =34) P value = 0.659 Not Significant Hypothesis Test #3: HRE is a function of hospital type Group 1(Teachng): (N = 11) Group 2(Non Teaching): (N =46) P value = 0.166 Not Significant Hypothesis Test #2: HRE is a function of hospital location Group 1(Urban): Median Income < $65,000 (N = 28) Group 2(Suburban): Median Income > $65,000 (N =29) P value = 0.548 Not Significant CONCLUSION The three variables hospital administrators believe to be strong causal factors of efficiency (size, location and teaching status) are not significantly correlated to efficiency for New Jersey hospitals Slide 9 Example
