Level Setting: Patient Family Engagement (PFE) Evidence is clear that patient and family is necessary for improving safety in healthcare Multiple tools exist to facilitate increased patient and family engagement but lack detailed supporting guidance for how to implement them. (https://www.ahrq.gov/research/findings/final-reports/ptfamilyscan/ptfamily4.html ) There are number of barriers to increasing authentic PFE * Consumers have a limited understanding of what patient safety means * The healthcare system is fragmented and difficult to navigate * Consumers lack knowledge and training to engage effectively Before we can authentically engage patients and families, they need to first be aware that it is necessary for their safety. Building Patient Partnership Capacity Awareness: Create a model of community engagement where we can bring awareness to patient safety and engagement to the broader community. Utilize an advisory group to give input on the process https://dementiafriendsusa.org/ Patient/Resident Engagement Training: Develop a curriculum to train consumers for active engagement at all levels of care across the continuum. Current Topics Informed consent – work in collaboration with the Health Literacy Partnership to explore interest in and willingness to improve process and forms Shared Decision Making – opportunity to join collaborative and play a supportive or leadership role Diagnostic Error - CTSI grant for pilot project in collaboration with Stratis Healht, MMIC, U of M and a community to test small changes in communicating test results to patients who visit the ER Open Notes – support awareness building work at Stratis Health
