Programme Board Aim ‘To develop an Urgent Care Resource Hub which coordinates well led and well supported multidisciplinary health and social care teams (including third and independent sector) to deliver urgent care in the Out of Hours Period’

Transforming Out of Hour’s Urgent Care thinkSpace Event An Introduction to our Programme

Background: The Ritchie Review In 2015, then Cabinet Secretary Shona Robison MSP commissioned a report into Out of Hours Primary Care in Scotland Sir Lewis Ritchie chaired the report, with the outcome of the report being 28 workable recommendations One recommendation is to create a multidisciplinary team/sector Urgent Care Resource Hub

Ritchie’s Proposed Model

Need for Change There are numerous pressures on current Out of Hours Services including workload, workforce and a move to deliver more care in the community Through the eyes of a patient, the Out of Hours landscape can be very confusing and often does not reflect the ‘right time, right care, right place’ model It is a vital service that needs to evolve and stabilise to meet the needs of the population

Programme Board Governance Programme Board Partners Allister Short (Chair) Accountable for strategic direction, sign off & finance Sarah Children Location & Capacity 8 Sub-groups Each Sub-group is led by a subject matter expert Palliative Care Communication & Technology Mental Health MDT Frail & Elderly Patient Awareness

The Event As the Programme Transformation Team, we want to hear how health care professionals and the third sector can work together to transform the out of hours urgent care service offered in NHS Lothian We aim to think outside the box to meet the health and social care needs of the population This will be a great opportunity to network and discuss how your organisation could support out of hours care delivery in Lothian
