Collectively we have achieved a lot (2014 – 2017) Together identifying our asset safety issues 6,065 inspections completed 33,901 Actions Raised - 29,633 Actions Closed 87% of all actions closed 92% of PIRs received to date in 2017 Water Industry Health and Safety Committee established. Huge investment in Asset Safety

A Real Story 38 year old man, married with 3 young children Went to work on the 23rd January Returned home 6 months later Broke multiple bones, punctured lung Severe crush injuries Intensive care Intensive rehab & physio

Key questions to ask yourselves as Leaders and Influencers Do you believe all accidents are preventable? Do you think that this could happen in your area? How confident are you that you have the full picture? Are the root causes identified and are you happy that you are learning from these incidents ? Are you happy that you are doing enough?
