mHealth for Chronic Dz Management Equip patients and/or families to capture real time data related to symptoms and quality of life indicators. EoE episodes linked to dietary triggers, thus we would like to capture meals, snacks and context via food diary Objective and subjective data documented between clinical encounters (vs. “usual” patient recall) Patient-provider communication enhancement Individual patient care will be improved Link data to electronic health record (EHR) Immediate impact: enhanced patient surveillance Long term plan: add mHealth intervention components over time Long term plan: multi-site collaboration (e.g., UNC-CH, Cincinnati)
