WEEK 2 – FIRST WEEK OF CLINIC Before session Submit updated/revised Lesson Plan or CHARTR section to your supervisor (supervisor will let you know which one to submit) – if you did CHARTR on your client, no separate lesson plan necessar  During session Ask client/family to fill out and sign required clinic forms Collect baseline data  After session Complete self-reflection and send to your supervisor – Observation reflection okay if you didn’t run the session Complete SOAP note (within 24 hours of the session) (Or read SOAP note if 2nd year is completing). Determine client/family goals, priorities, and expectations for term Discuss LTGs/STOs, measures, and data collection approaches with your supervisor Complete CHARTR Worksheet
