Environmental Health and Safety | University of Washington Accessing OARS Login to the OARS system using the address below: https://oars.ehs.washington.edu/Oars/LoginUWashingtonShib Any student, staff, or faculty member can file an accident/incident report. Accident reporting is required by federal and state laws as well as University policies. Environmental Health and Safety | University of Washington Page 1 Indicate in what capacity you are filing the report. Select the appropriate option, either as supervisor, self, or university representative. 2) Select the status of the person injured or involved in the incident, e.g. Faculty/Staff or Graduate Student. 3) Select the individual involved by clicking “Select Involved.” Search the UW directory for the individual. Once found, click “Select Person.” 4) Select the supervisor of the person involved by clicking “Select Supervisor.” Search the UW directory for the individual. Once found, click “Select Person.” 5) Once Page 1 is complete, click “Next” on the bottom left corner of the page. 11/18/2018 Environmental Health and Safety | University of Washington

Select the campus the incident occurred and then click “Select” to specify where the incident occurred. You can search for the location by leaving the fields blank or entering the whole or partial name of the building, address, parking lot, etc. Once found, click “Select Location.” Give a detailed description of the incident; the more information, the better. You can also attach supplemental materials on this page, such as photos. Once Page 2 is complete, click “Next” in the bottom left corner of the page. 11/18/2018 Environmental Health and Safety | University of Washington

Environmental Health and Safety | University of Washington Pages 3-4 Page 3 requires the incident to be classified by severity, nature of injury, body parts affected, and cause of harm. Once the appropriate options are selected, click “Next” on the bottom corner of the page. Page 4 requires more information on the causes of the incident. Select the appropriate contributors to the incident as well as suggested corrective action by the affected party. Note: It is important to take the time to accurately check the boxes on pages 3 and 4. This information is a large source of the statistical data that is created for OARS reports. 11/18/2018 Environmental Health and Safety | University of Washington

Page 5 – Completed by Supervisor Detail the root causes of the incident. This information should come as the result of a thorough investigation into all of the factors that could have contributed to the incident. The recommendations/preventive measures are the suggestions and plan of action to be taken by the supervisor and/or department to ensure a similar incident does not occur again in the future. 11/18/2018 Environmental Health and Safety | University of Washington

Page 6 – Completed by Supervisor Select a target date for the corrective actions outlined on Page 5. This is the date by which you expect the actions will be completed. Acknowledge that you approve the investigation and corrective actions by selecting “Yes.” Once the corrective actions are completed, enter the complete date. Indicate that the actions are complete by selecting “Yes.” If the report is complete, it is important to click “Submit Completed Report” on the bottom right of the page.
