QUESTIONNAIRE Basic Statistics on you Unit or Club How is your Membership doing? Age statistics, residence location, active or inactive, What are you doing to attract quality members? Who do you foresee as your future leaders? Are they ready to lead effectively, or not? 11/18/2018

QUESTIONNAIRE – 2 Do you have a Web Site? Is it linked to the Temple Web Site? Is your Web Site effective? How do you know if it is effective? Who is your web master? Is he paid? Is the present arrangement working? What is your backup if he/she leaves? 11/18/2018

QUESTIONNAIRE - 3 Do you maintain a list of members on line? Do you make it easy for someone on-line to understand how to join your group? Do you include petitions for Blue Lodge, Shrine, and your Unit/Club on-line, and contact information for each entity, on line? How many hits to do you have your Petitions pages? QUESTIONNAIRE - 7 What does your Unit/Club do for Fraternal Fund-raising for itself? Has that been successful? Do you plan to continue doing it? Temple Fraternal Fund-raising? Fund-raising for Shriners Hospitals? Fund-raising for Transportation Fund? Can you do more? If so, what would that be? 11/18/2018

What can your Unit/Club do to support the local community with manpower? QUESTIONNAIRE - 13 OTHER EVENTS AT GLSA: Golf Tournament with trophy Cruise-in for special vehicles Pistol Shooting Contest with prizes and trophy Community Parade with thousands of spectators Afterglow inside the Hotel after the Parade Ceremonial after the Afterglow Formation of new Unit Associations: Chanters, Band, Drum & Bugle Corps, Networking, Cast, Directors. What other Unit Associations would you like to see? 11/18/2018

QUESTIONNAIRE – 14 ANSWERS: Your answers will be tabulated and presented at the April 2 Leadership Academy. The author of the best answers will be called upon to provide details to the rest of the attendees. A summary of the answers will be provided to each GLSA Potentate for use by their Temples. In the GLSA Sept. Business Meeting each Temple will be asked to describe what changes or progress has been made based on this Questionnaire. So: TURN THEM IN to Phil Thrasher. 11/18/2018
