Improving Health Care Outcomes for East African Communities Presented by: jeri Peters, Chief nursing Officer RUSS 2013 Age and Disability Odyssey: June 17, 2013 4pm-5:15pm Split Rock 2, City Side

Objectives Describe the challenges faced by East African communities in accessing health care in our country. Identify the elements of strong partnerships that incorporate the role of funder and health plan as strong partners in serving the East African and immigrant communities. Provide an overview of several examples of initiatives that improve the health outcomes of East African communities and that provide access to and achieves better health outcomes by promoting early intervention and partnership models using community health workers, public health nursing and transcultural nurses.

UCare Mission UCare will improve the health of our members through innovative services and partnerships across communities. Founded in 1984 by faculty of the Department of Family Medicine at the University of Minnesota Medical School. Began in response to Minnesota’s Medicaid Managed Care Demonstration Project. Initial purpose: to support the DFM residency clinics in a managed care environment and provide quality care to low-income and diverse populations. At nearly 300,000 members, 600 employees, and the largest provider of health care services to State public program enrollees. UCare exists to provide comprehensive health care to selected populations through a community-based, holistic approach, and to support family medicine education and research so that this philosophy of care can be carried on into the future.
