How to register patients via EEG services Once you have logged in as an EEG user (see user guides - user types and how to request to register as a user), please use the following steps on how to register patients on the Epilepsy12 portal. NB All data displayed within this guide is for fictional purposes. Step 1 From the homepage click the ‘EEG clinical audit’ tab and select ‘EEG registrations’.
How to register patients via EEG services Step 2 The Initial Patient Registration form record management will be displayed. From here you can begin to add patient registrations by clicking ‘Add record’.
How to register patients via EEG services Step 3 The Initial Patient Registration form will open and you can begin entering patient details within the displayed fields.
How to register patients via EEG services Step 4 All fields within the Initial Patient Registration form are mandatory and require complete data entry. When you click the ‘Save’ button on the bottom right hand-side of the screen any missing data fields will be flagged as “Required”.
How to register patients via EEG services Step 5 Once all the fields within the Initial Patient Registration form are complete, click the ‘Save’ button. The form will go green to indicate the fields are complete, then click the ‘Submit record to Trust’ button on the top left hand-side of the screen. Be sure to indicate the correct referring Health Board/Trust
How to register patients via EEG services Step 6 Once you have submitted the registration form you will taken back to the Initial Patient Registration form record management screen. You will find the patient you have registered displayed within the table. Repeat steps 2-5 to add additional patients.
