Epilepsy12 round 3 data platform How to register patients as Health Board/Trust users

How to register patients via Trust services Once you have logged in as an Trust user (see user guides - user types and how to request to register as a user), please use the following steps on how to register patients on the Epilepsy12 portal. NB All data displayed within this guide is for fictional purposes. Step 1 From the homepage click the ‘Trust clinical audit’ tab and select ‘Clinical audit’.

How to register patients via Trust services Step 2 The Clinical Case Management screen will be displayed. From there you can both verify the patient registrations that have been added by the EEG services or you can register Trust patients yourself via the ‘Add Record’ button.

How to register patients via Trust services Step 3 Once you choose to add a record, the Initial Patient Registration form will open and you can begin entering patient details within the displayed fields.

How to register patients via Trust services Step 4 All fields within the Initial Patient Registration form are mandatory and require complete data entry. When you click the ‘Save’ button on the bottom right hand-side of the screen any missing data fields will be flagged as “Required”.

How to register patients via Trust services Step 5 Once all the fields within the Initial Patient Registration form are complete, clicking on the ‘Save’ button will turn the form header green. The next step is to click on the ‘Trust Verification form’ tab and continue to complete that aspect of the form.

How to register patients via Trust services Step 5 Once all the fields within the Trust Verification form are complete, click the ‘Save’ button. The form will go green and the ‘Submit and lock record’ button will become available on the top left hand-side of the screen.

How to register patients via Trust services The Clinical Case Management screen will also list the patient registrations that have been added by the EEG services that require Trust Verification. The registrations will appear as Registered under the Verified column and Incomplete under Record status. To verify patient registrations, click on the ‘Pencil Icon’ on the right hand-side.

How to register patients via Trust services The Initial Patient Registration form will open up with all fields already completed by the EEG Service other than Q 1.10. To verify that the patient was correctly indicated as referred for the EEG by your Trust, click ‘yes’ to question 1.10 and save the form. Then continue with completing the Trust verification form. If the indicated patient that was registered by the EEG Service was not referred your Trust, select ‘No’ to Q 1.10, ‘Save’ the form and contact the Epilepsy12 project team via email (displayed on the last slide of this guide) quoting the case ID only. The project team will then be able to delete the registration and ask the EEG Service to check which Trust referred the patient for the first EEG before registering the patient onto the system again.
