2018 HealthQuest Rewards Same as 2017 Plan A – Premium Incentive Reduction Plan C, J, Q, N – Premium Incentive Reduction as well as HSA/HRA Contribution

Premium Incentive Discount for 2019 HealthQuest Rewards Premium Incentive Discount for 2019 Employees & covered spouses that earn 40 credits each. Discount earned by coverage tier: EE & EE/Children Tiers: EE can earn the full $480 of the premium incentive EE/Spouse & EE/Family Tiers: EE & spouse can each earn $240 of premium incentive discount
