Database Committee Report Infection Organism Mapping Infection organisms have been a free text field since data collection began in 1993 Hundreds of unique variations of organisms have been entered Committee reviewed and mapped all organisms to a series of check boxes to make data more ‘useable’ for research and analysis Removed infection ‘type’ to prevent data entry errors

Database Committee Report Current Infection Organism Data Collection

Database Committee Report Infection Definition Previous Definition: Evidence of infectious process requiring IV therapy or life threatening requiring oral therapy. New Definition: Is this infection a severe infection that requires IV therapy, hospitalization for infection >2 days, or escalation of care in an already hospitalized patient?

Database Committee Report Annual Follow-up Medication Mapping Annual Follow-up medications have been a free text field since data collection began in 1993 Thousands of unique variations of medications and spellings have been entered Committee reviewed and mapped all medications to a series of check boxes to make data more ‘useable’ for research and analysis Mapping is under final review by the committee and will be implemented in the electronic system soon.

Database Committee Report Annual Follow-up Medication Mapping Medications will also carry over from previous follow-up for coordinator to review and confirm form to reduce data entry burden of entering medications again each year. A ‘Medication Look Up Table’ will be added to the manual showing what was mapped to each category for coordinators to reference and reduce the data entered into ‘other, specify’.

Database Committee Report New Medication Categories Anti-hypertensive Mycophenolate (Cellcept, Myfortic) Azathioprine (Imuran) Pneumocystis Prophylaxis Cytoxan (Cyclophosphamide) Prednisone Cyclosporine Sirolimus (Rapamycin) Diuretic Statin Everolimus Tacrolimus (Prograf, FK506) Immune globulin Other, specify Methotrexate
