Arterial pH and TCO2 will be low and the anion gap will be high. The primary toxic effect of acute salicylate overdose is severe acid–base disturbance. This effect is mediated by the acidic nature of the drug, a stimulation of lactate and ketone production, and a direct stimulation of ventilation, which contributes to a respiratory alkalosis. In high-level overdose, the net effect is an apparent metabolic acidosis. Arterial pH and TCO2 will be low and the anion gap will be high. Metabolic acidosis is the most serious acid-base disorder and is due to increased production of endogenous acids rather than the salicylate itself. the drug uncouples oxidative phosphorylation
