Community Reorganization Plan Vision: To transform current system of separate primary mental health and addictions services into integrated, interdisciplinary, geographically-based teams Goal: Develop interdisciplinary teams in four (4) geographic locations within the metro area with strong connections with psychiatry Timeline: Early March, 2017 Service Model: Stepped Care Approach Recommendation for development of community based interdisciplinary teams cited in the ‘Working Together for Mental Health – a regional action plan for Eastern Health – July 2005 Vision and service model were supported by the All Party Committee Recommendations. Planning for the reorganization started in early 2016 with the formation of a steering committee with representation from Management and the affect clinical programs.

Clients may move up and down the steps throughout their treatment at Eastern Health.
