State Health Plan Open Enrollment 2017

Open Enrollment Dates and Instructions Open enrollment begins October 1st, 2016 and ends October 31st, 2016 All employees INCLUDING NEW HIRES have to go through the open enrollment process ALL EMPLOYEES WHO DO NOT CURRENTLY HAVE INSURANCE, OPEN ENROLLMENT IS YOUR ONLY OPPORTUNITY TO ENROLL. IF YOU DO NOT ENROLL, LEE COUNTY SCHOOLS, WILL NOT BE RESPONIBLE FOR YOUR INSURANCE.

The State Health Plan has three plan options for 2017 Consumer-Directed Health Plan 85/15 (CDHP) Enhanced 80/20 Plan Traditional 70/30 Plan Wellness Premium Credits will be offered to reduce employee-only premiums. These credits will apply to the CDHP 85/15 and 80/20 Plan, and NEW this year to the 70/30 Plan

Overview of Changes: Enrollment Process ALL members and eligible dependents will default to the Traditional 70/30 Plan – effective January 1st, 2017 If members want any Plan other than the 70/30 Plan, they MUST take action and enroll in their preferred Plan. If a member DOES NOT take action by Oct. 31st, 2016, they will remain in the 70/30 Plan and pay a premium.

Change Comparison Charts for 2017 Listed on the next two screens are the comparisons for the three Plans offered by the State Health Plan for the year 2017. Please look over these screens VERY carefully. There are many changes taking place in the year 2017.

Directions on Enrollment Continued Click on the Get Started tab Make sure you are on the Open Enrollment Benefits tab, NOT Current Benefits The box will show your 2016 Medical Coverage, click on the Edit Plan tab On the next screen make sure you click on the Open Enrollment button and click next

Directions on Enrollment Continued REMINDER: All Three Plans (this includes the 70/30 Plan) have the Tobacco User Attestation – Please be sure to answer this to receive your premium credit The 80/20 Plan and the Consumer Direct Plan have the Primary Care Provider and the Health Assessment in addition to the Tobacco User Attestation – Please be sure to answer these to receive your premium credits for these plans.

A Note from the Health Benefits Representative VERY IMPORTANT: Open enrollment will be held October 1st-31st, 2016. Click on the plan in which you would like to enroll. Remember to click SAVE when you complete you enrollment and your Wellness Premium Credit. Print your Benefit Detail Report and save a copy for your records.
