Five week Programme/Outcomes Decided upon by patients Challenge dependency/ adequate support What was important to the patients? 10-12 participants with caregivers 6-12 weeks post hospital discharge

Physiotherapy (Week One- Three) One to One Appointment What matters to the patient? Building on Personal Goals Chronic Pain issues 66% of patients- new pain since ICU admission 72% of this pain in lower limbs and shoulders

Pharmacy (Week One to Three) One to one Appointment Education Empowerment Medicine Reconciliation The pharmacist identified pharmaceutical care issues with 18.3% of the prescribed medications 57% of these interventions increased the therapeutic benefit and potentially avoided significant side effects

Clinical Psychology (Week Four) Diverse needs Focus on coping skills Group session Sleep session Patient and caregiver sessions Significant ongoing problems for both patients and caregivers

Social Prescription Week (Week Five) ALISS Employability (including options for volunteering) Good Moves Carers Centres GP CAB Increased employment in our cohort in comparison to a previous group from GRI (88% vs. 46%)

Overall Results Improved self efficacy over the five week programme and one year period. Magnitude similar to pulmonary rehabilitation. Signal to improved quality of life compared to a historical control group. Qualitative findings demonstrating patient satisfaction and acceptance with the intervention.

What about the carers? Carers are involved in all of the interventions Can take part in the exercise class Have undertaken volunteering roles Qualitative findings found benefits in this approach
