Justification If an endorser makes any changes or additions to any causal factor conclusion, RAC, or recommendation he/she will do so and provide detailed analysis as to why the changes were made. The endorser is afforded the opportunity to add, restate, or delete any portion of the previous endorsement

Endorser’s Final Comments.

At this point, the Endorsing Chain should have already been established by the Controlling Custodian. However, in certain circumstances an Endorser may have to “add” an additional endorser IAW the OPNAV 3750.6 (series) Instruction.

Reconvene Option Available Reconvene Option Available. Any endorser who finds an investigation incomplete, or SIR inadequate, may reconvene the AMB and direct them to address the specific areas of concern. This must be done via WAMHRS infoing all concerned. The endorser who reconvenes an AMB must detail why he/she is reconvening the board and provide specific guidance as to what the AMB should address.
