Prospective Member Form Name: Arnold (Arnie) Banks Occupation: Retired Birthday: May 15, 1948 Address: 501 W. Main St. Louisville, KY 40202 Spouse: Lydia Banks, age 57, still employed as a teacher at Jefferson County Public Schools Children: William Banks, 42; Grace Banks Santiago, 40 Agent notes: Met Arnie at the local YMCA playing racquetball. He mentioned that he had recently retired and is looking to enroll in a Medicare plan with PDP Coverage, but doesn't know where to start. Referred him to our online options, and he submitted a request to meet with an agent. Additional information: Arnie is above the income threshold to get Medicare subsidies. His past employer does not offer him Medicare benefits. His wife will keep separate insurance for now. He’d like to keep his same local retail pharmacy, CVS downtown for acute needs, but is open to using the Humana pharmacy or mail-order for his maintenance meds. He is open to using generic medications. He’s an established patient at the Partners in Health practice and prefers St. Michael’s hospital downtown.
