Why Integrated Care? Utilise the expertise of all providers (prudence) Increase capacity to meet demand Bring Care Closer to Home Work together to improve patient outcomes and experience

Glaucoma ODTCs in all unitary authority areas within ABuHB Reduce delayed follow ups and associated clinical risks (first 12 months 47% reduction) Up to 4,000 patients per annum
