Technology Enabled Care Model The Lutheran Home Association expanded the Technology Enabled Care model through funds awarded through the MN- DHS Community Service and Service Development (CS/SD) grant projects. The CS/SD grant projects are led at the state level by Jacqueline Peichel, Aging HCBS Development Lead, MN Board on Aging, MN-DHS. Grant funds received through the MN-DHS CS/SD grant project have supported the implementation of the Technology Enabled Care model and educational outreach programs for the community This unique model utilizes health monitoring technology paired with an individualized service delivery system
