Forms & Fees - all ONLINE physical – on hand or no participation, practice or game all purpose/insurance form – essential for medical emergencies Code of Conduct - concussion form – verifies watching concussion video Medical release – completed with athletic trainer, Doug Nickels fee - $50; $125 max/athlete; $250 max/family hat - $5/athlete, $20/other(availability) player/parent code of conduct – verifies receiving and reading player handbook

Equipment supplied by MHS helmets, bats, baseballs, uniform, hats($5), catchers gear Equipment supplied by athlete(school colors) cleats(school or summer colors), black undershirt, glove, cup, black stockings, black belt, pants & sleeves to practice in, cold weather gear

MAR Club next meeting – April 9th, 5:30 pm in MHS library sponsor AOM, AOY, seasonal team kickoff & meal spring kickoff – April 9th, 7pm in MHS gym(all athletes expected to attend) parents, grandparents, fans invited also spring MVA banquet(April 30th, 6:00 pm in MHS gymnasium by invite only)
