Man crushed by falling BOP - FATALITY HSE ALERT Man crushed by falling BOP - FATALITY Rig hands were preparing an adaptor spool for mounting a single BOP. Before the preparation was complete the forklift arrived at the scene carrying the BOP. The operator kept the load suspended approximately 1 metre off the ground close to the workers for around 15 minutes. Over the time, the forks slowly drooped down until eventually the BOP slid off the forks. One man was struck on the lower leg and suffered a deep cut to his calf. A second man was trapped and crushed with both legs under the BOP. The second man died later as a result of his injuries. The accident would have been avoided simply by lowering the load to the ground whilst waiting for the preparation to be completed, or by waiting a safe distance from the work. Poor planning (lifting the BOP instead of adaptor) created a higher risk job. Reconstruction
