What Makes for Success? A well prepared team Knowing “where” you are (e.g., how did we do this past year?, what does our final reporting show?) Access to data Ready to “hit the institute running” Having a facilitator!

Preparing Your Team Build a team that includes key personnel Pull your team together before the institute if possible Acclimate new members to the institute process Identify team member roles if you haven’t already

Know Where you Stand Use your transtionprogramtool.org site Did we complete our tasks? Did we produce our outputs – our products? Did we collect and review data relevant to our expected outcomes? Did we achieve our outcomes?
