Factors influencing engagement Feeling unprepared for the level of responsibility (isolation, stigmatisation etc.) Lack of relationship with new clinical team Insufficient understanding of disease Inflexible services Difficulty identifying/accessing support Finding the balance between life & disease management

Aim: Explore and scope the feasibility of a fit for purpose peer supported diabetes self-management programme for young adults (16-25 years) Approach: Build stakeholder involvement – local council, employers, education institutions, food academy etc. ‘Normalise’ diabetes management (re-branding i.e. less clinical) Create an all-inclusive model for family, friends and carers Bi-monthly groups within community venues and social settings (Leisure centre, creative-arts centre, bowling, go-karting etc.)

Progress Future intentions Outcomes To increase membership To develop a sustainable model for group To monitor clinical outcomes of attendees Collaborate with East Ham Community Neighbourhood Team to integrate peer support group into existing youth hub at East Ham Library. Initiative to be launched October 25th Outcomes Improved relationship with HCPs – co-creation of initiatives & format Improved confidence in decision making Greater awareness of services Improved confidence in use of technology for clinical care Established successful collaborations with multiple organisations DUK collaboration – Type 1 London event (June 11th) Production of peer support videos (funded by The Health Foundation) Production of a quarterly newsletter – Youth Commissioning Champions Appointment of a Youth Champion – funded by Newham CCG

I enjoyed being here and will do it again. As an observer, you need to share experiences from the activities so other people at the clinic see what’s happening. (Friend) We all had a good day and bonded. We made friends with who have gone through the same things. Great fun, heart pumping karting! Amazing to meet other diabetics and just have some casual fun  I enjoyed being here and will do it again. It’s a group that families and friends should know about. I have enjoyed this group. (Parent) It’s nice to talk to people that are in the same boat. I think there should be more people talking about this group.
