Cervical Spine 98940(1)(2) tells the office and the claims adjuster that manipulations were done on the spine in 1 to 5 regions. If only the cervical box is checked, the office will bill 98040. If there are boxes checked in cervical, thoracic, lumbar and pelvis/sacrum, the office will bill 98942 (5 regions, since pelvis and sacrum are 2 regions)

3x the Medical Bills? Colossus doesn’t even figure in the medical bills when determining the settlement offer. What happens is, all the points (especially with the multipliers) are added up, then a dollar value is figured based on the points, then the medical bills are simply added on at the end (a 1x factor). Anxiety & Depression Must be “treated” Colossus considers them “treated” if the patient does exercise, medicates, avoid stressful activities, and rests in bed. Hmmm… home exercises, gym exercises, and in-office exercises can be used as a 3x multiplier for nearly every injury the patient has.
Cane/Orthotics Big points are awarded if the doctor prescribes the use of a cane, crutches, or orthotics. If the patient has a “difficulty walking” diagnosis, any of these might be useful. Orthotics are easy to prescribe and use. It is a form of “stabilization”
TENS Units Big points are awarded when the doctor prescribes a TENS unit for home use. If used 7 days a week, it is “intensive.” If used >90 days, it is “prolonged.” At $50 wholesale cost, sell them at $75 to $100. It’s not the profit in selling it you care about… the case settles faster and for more money. Your lien gets paid much faster.
TMJ This is “extraspinal”, so if you adjust the TMJ, it is a 98943 If temporalis, buccinator, or pterygoid massage is done, that is both physiotherapy and massage therapy. A $70 mouth splint is used 7 nights a week (intensive) for >90 days (prolonged) 3 x 3 = 9x value Soft food diet is also “restricted TMJ activity, so 2 boxes get checked when you give the patient these home care instructions (see www.HBTinstitute.com)
In Summary… Introduce “active” therapies ASAP Document on the Treatment Plan form how often to do the “home” therapies Document on successive Treatment plans the continued instruction to keep using them. Use fewer “passive” therapies and more “active” ones.
More Summary… The Treatment Plan Form is filled out at first exam, each re-exam, & final. There will be 3 to 6 of these when you are ready to write the narrative report. Make sure they include “treatment” that patient will continue to do at home after the Doctor stops treating the patient in the office.
