SIRS Systemic Inflammatory Response Syndrome Building Blocks of Knowledge

SIRS Building Blocks of Knowledge SIRS is nonspecific Bacteremia ACCP offered this definition of SIRS: Fever > 100.4 OR < 96.6 HR > 90 RR > 20 OR PaCO2 < 32 WBC >12, <4, or 10% immature Can be caused by: Ischemia Inflammation Infection Trauma Or a combination of insults Is the presence of bacteria within the bloodstream, but does not always lead to SIRS or Sepsis. Sepsis is the systemic response to infection and is defined as the presence of SIRS in addition to a documented or suspected or presumed infection.

SIRS Building Block of Knowledge Infection triggers an inflammatory response * Redness, swelling heat and pain are present locally If homeostasis is not achieved, the inflammatory response will cross over into the circulation, and a significant inflammatory reaction begins to cascade

SIRS Building Blocks of Knowledge The consequence of this is a loss of circulatory integrity due to cell destruction * Capillary’s leak, microvascular thrombus develop, tissue hypoxia, cell death, impaired vascular … Leading to Multiple Organ Dysfunction

SIRS Building Blocks of Knowledge Multiple Organ Dysfunction Syndrome is a process, not an event Characterized by the development of progressive physiologic dysfunction in two or more organs or organ systems No organ system is immune Mortality increases as organ failure increases Mortality ranges from 40-75% in patients with MODS arising from sepsis
