CHHHS NURSE NAVIGATION SERVICE THE NURSE NAVIGATOR …. Is a senior nurse that has system knowledge, skills and clinical expertise …. Each Nurse Navigator has been assigned a cohort of patients; that warrant an additional resource due to the complexity of their health needs … Can provide a helicopter view point of the patients health journey (holistic) PRINCIPLES OF NURSE NAVIGATION (NN) …are to integrate this NN health care model as part of a multidisciplinary team, and complement existing CHHH models of care …will identify gaps in care and provide recommendations re: system process changes; modifications to improve the patient journey …will bridge the gap between hospital to community to ensure the patient with complex needs is not lost in the system …will support the patient in regards of health literacy and self management of their healthcare …will identify, assess and review patients with complex needs and will work in partnership with the patient to develop a care plan Trudi Jobling CNC Nurse Navigator Paediatrics NN4kids-CHHHS@health.qld.gov.au; phone 42268611 mob 0419347121
