Dictate H&P within 24 hours of admission Dictate OP/Procedure note immediately after a procedure Use Meditech e-signatures Use Meditech to review your incomplete record list Dictate or electronically sign records to complete them at least 9 days BEFORE they become delinquent (to allow time for transcription and your e-signature) Contact Medical Records Staff if going on vacation or out of town Do all deficiencies each time you complete your records Always let us know as soon as possible if there is a discrepancy in who is responsible for the record
Result of Not Completing Records Timely Physicians are called daily for missing H&P or operative notes. Every week a notification of incomplete records is faxed – the letter is available in Meditech messages. Records requiring dictation should be dictated within 21 days and must be complete including authentication within the 30-day timeframe. Medical Affairs contacts physicians who have not completed records within 21 days. Failure to complete delinquent records within 30 days of discharge triggers automatic suspension of clinical privileges. Suspension status will remain in effect until all delinquent records are completed. A physician who has been suspended six or more times within one year (rolling 12 months) shall have voluntarily resigned from the Medical Staff.
