Make clear what the regular program is by including 5 digit registration number Get Governance Approval Prepare a Request for Change in a Currently Registered Program Form, using the form as a guide. Make sure to include a side-by-side Comparison of the entire old and new Curriculum (from/to). If applicable, include appropriate curricular or faculty information from the following Curricular Include syllabi for new courses and catalog description of old courses Faculty SED Form Plus applicable CV’s (max of 2 pages) Mail, in duplicate, to the Director of Program Review, Articulation and Transfer with CUR Month and Item Number
