1. Select California State University from the drop down selection

Enter Participant’s SSN (9 digits no dashes) Enter Participant’s Last Name Enter Participant’s Date of Birth (mmddyyyy- no slashes) Click Submit

The Participant will create a new password based on the password criteria to the right. Passwords are case sensitive. Example of a Password: Pass1word 2. Enter the same password in the Re-enter Password field 3. Click Enter
