SAFE WORK INSTRUCTIONS Title of Subject: Lo Trac - Mini Trac Last Reviewed Date: December 2015 SAFE WORK INSTRUCTIONS Steps for Initial SWI Training: Prepare – Review the SWI. Present – Quick Card at the machine with experienced operator. Tryout – Person runs the machine with supervision. Follow up – Competency points are verified by supervisor and 5000-23 is signed by the trainee to complete the training. Steps for Retraining: Review SWI or Quick Card Verify Competency Points Complete Annual Training Requalification Form Competency Points: Pre-op © Tramming © Shut Down   The following safe work instructions is NOT INTENDED to replace hands on training requirements of the code of federal regulations. The intent is to provide an overview of known potential safety hazards associated with equipment operation. Hands on training with an experienced operator on each specific model must be conducted. The trainee can not proceed onto production until they are determined ready to proceed by the supervisor of the training. Before any operation or maintenance procedure is performed with or on this vehicle, it is important that personnel have a complete understanding of the location and functions of all controls and indicators, as well as, the location and use of all emergency shutdown controls and procedures. SAFE WORK INSTRUCTIONS Title: Lo Trac - Mini Trac SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Pre-Operational Checks Examine work area, roof, ribs and travelway Check switches, levers and foot controls. Check start and stop controls, e-stops , air filters, fuel leaks and oil leaks Check attachments ,bucket, forks for proper connections and wear Scale roof and ribs and maintain a clear travelway at all times All levers shall be centering and all switches in the OFF position Report any malfunctions to foreman or mechanic. Tramming / Operation All persons are to be in the clear, sound warning device and no one is allowed to pass while it is motion. Control speed and direction with foot pedals and self locking hand throttle When ascending or descending on a ramp, slope or incline always keep the heavy end of mini trac up hill; if bucket or forks is loaded, keep bucket uphill; if empty keep attachments downhill When changing attachments ,always lower attachments ,turn machine off and block against movement before removing any pins or keepers Sound warning device before start up and travel, and never let anyone enter red zones Lights shall be used in direction travel Always maintain a safe operating speed Never cross under raised attachments Ensure loads are properly secured before traveling. Keep bucket low to surface when traveling Shutdown Never park on a incline or grade and block wheels against movement Lower forks or bucket and tilt so that bucket or forks rest firmly on the ground Set park brake, turn engine off Blocking wheels prevent unexpected movement of machine Place bucket levers in float position to prevent unexpected movement when dismounting or mounting machine Knock main power switch in engine compartment when finishing operating Never leave machine unattended while running
