Video Release Form AKA- What is it? Used For… When Must u Use One? Appearance Release Form What is it? a simple contract that gives you legal permission to use the image of the person who has signed the form for commercial and non-commercial purposes Used For… Protects your rights; Keeps you out of trouble; and, Covers all the points related to your situation. When Must u Use One? If you plan to use a person's image for commercial purposes, you need to get a signed video release form from that individual. If a person is clearly identifiable… get them to sign a release General Exception Shooting a crowd w/ 10 or more people

Video Release Form You will want to get a release form if… shooting a video, and you interview someone or shoot footage of an instructor. shooting with a audience, and audience member appear on the video - esp. if you use clips of identifiable audience members Shoot video at a private event, or within a place a business, or a home – you will also need to get permission from the owner or organizer of the event in addition need video release forms from each person you tape.

Video Release Form A good video release form should… Specify what is being released (video images, audio, photographs). Specify that the videographer may sell or assign the right to use the images or other materials to third parties. Specify that the release cannot be changed or altered. Is signed and dated by the individual releasing his/her images. If the subject is a minor (under 18 years old), the release should be signed by a parent or legal guardian. Is in writing.
