IMPORTANT Registration Information Registration Cost- $15 WHEN: Friday, February 17, 2017 WHERE: Lexington Technology Center 2421 Augusta Highway,  Lexington, SC 29072 Tentative Agenda Registration 8:00 – 9:00 am Competitive Events 9:00 am -1:00:00 pm Awards Luncheon 1:00 -2:00 pm Students who place in the top 3 are eligible to compete in the  2017 International Conference in Orlando, FL in June.

Log into your SC HOSA account

Scroll down and click to “Local Chapter Advisor”, then to “Access Local Advisor Services”

Type in Chapter Charter # and Password and select “Login.”

Select Conference Registration.

Select 2017 SC HOSA Middle School State Leadership Conference from the dropdown list and click “Begin Registration.”

A complete list of your HOSA Members will appear. Beside each member, there is a link to register that member for the conference. Click “Register” for the HOSA member you want to register. You will then see a screen with all of the registration options.

You will need to provide the following information for EACH person you register: Select the appropriate GENDER Under TYPE, select Middle School Advisor Professional PLEASE MAKE SURE YOU ENTER THE CORRECT “TYPE”, this could affect their competitive event. Select the tee shirt size for each member/person registered (students, advisors, chaperones, etc.) Select the competitive event and recognition event(s) in which the student will Participate. Limit of one competitive event per student.

Click “SUBMIT” after entering each person’s registration information.

If you need to register a family member, chaperone or other professional, click “Register Family/Guest/Other”

Enter all information for your Chaperone/Guest/Other here.

Forms needed for everyone attending the State Conference: Medical Forms, for Students, Advisors, Chaperones and Guests. Once you have all of your forms from all of your students, advisors, guests and chaperones, submit them through our Middle School Forms link by February 13, 2017. Code of Conduct Forms Advisor/Chaperone/Guest Code of Conduct Student Code of Conduct (Students, Advisors, Chaperones and Guest must have a copy.) Please submit your forms through our Middle School Forms link by February 13, 2017.

After completing all registrations, save a copy of the Summary Report/Invoice, for your files.
