I’M ALSO CO- CHAIR OF THE ICMHA OUTREACH AND CLINICAL SERVICES WORKGROUP I’M GOING TO REVIEW SOME OF THE EFFORTS THAT HAVE BEEN TAKEN UP BY THE ICMHA AND THEN SCOTT KLOBERDANZ AND RACHEL ROBERTS WILL COME UP AND TELL US ABOUT A SERVICE DELIVERY MODEL THAT HAVE BEEN CREATING

PRIMARY GOALS Promote mental wellness among aging Iowans Increase access to qualified mental health service providers Integrate mental health services nto usual places of care LAST SPRING WE IDENITIFED THREE GOALS THAT PERTAINED TO OUTREACH AND CLINICAL SERVICES….

I am a psychologist who services the geriatric population. Can you Ask the Expert I am a psychologist who services the geriatric population. Can you direct me to resources which are related to the use of psychotherapy with mildly demented patients? Would like Screening questions to be used on an acute care hospital nurse admission history to screen for suicide. Would appreciate an evidenced based tool that is quick and easy to use. I am working with a client who is exhibiting symptoms of depression; what is the age range criteria to get an adequate score with the Geriatric Depression Scale? I am a licensed Psychologist who works in nursing homes and long term care facilities with geriatric patients. What short screening tests might you use to measure anxiety with this population? WE LINKED THE ICMHA WITH THE IOWA GERIATRIC EDUCATION CENTER WHICH HAS CREATED A POOL OF CLINICAL EXPERTS WHO OFFER DIRECTION TO PERSONS WHO ARE PROVIDING MENTAL HEALTH SERVICES TO OLDER IOWANS…. THESE ARE SOME EXAMPLES OF THE QUESTIONS THAT THE EXPERTS HAVE BEEN ASKED….

Practice Network CMHA Coalition Members - April 2006                                                                                                                                                                 WE ALSO HAVE CONTACTED SEVERAL PROFESSIONAL PROVIDER ORGANIZATIONS SUCH AS THE IOWA PSYCHOLOGICAL ASSOCIATION AND IOWA NURSE ASSOCIATION AND HAVE SOUGHT TO IDENTIFY INDIVIDUALS WHO ARE CAPABLE AND INTERESTED IN PROVIDING CARE TO OLDER IOWANS WITH MENTAL ILLNESSES. AS WE COMPILE THESE LISTS, WE WILL PUT THEM UP ON THE ICMHA WEBSITE SO PEOPLE CAN FIND A PROVIDER IN THEIR AREA….

WE ALSO HAVE CONTACTED THE IOWA PEER SUPPORT TRAINING ACADEMY AND HAVE REQUESTED THAT THEIR ALSO OFFER PEER SUPPORTS TO AGING IOWANS NOT JUST CHILDREN AND YOUNGER ADULTS….

COLLABORATIVE MODELS Primary Care Practices Nursing Homes & other LTC facilities Primary Care Practices FINALLY, MEMBERS OF THE ICMHA HAVE BEEN SUPPORTING EFFORTS TO EXPAND AND IMPROVE MENTAL HEALTH CARE IN NURSING HOMES AND OTHER PLACES

Nursing Homes Nursing Home Quality Initiative IFMC, Iowa’s Medicare Quality Improvement Organization (QIO) offers assistance at no cost to nursing homes by providing education, identifying improvement opportunities and sharing valuable resources and best practices. The focus of IFMC’s Nursing Home Quality Initiative is to assist nursing homes in providing the best possible care for their residents. These services include improving care processes and setting improvement targets for pressure ulcers, restraints, depression and chronic pain. Additionally, IFMC is working with nursing homes to increase resident and staff satisfaction through work on organizational culture change and implementing person-centered care. Our Nursing Home Quality Initiative combines this assistance to improve quality of care with information for consumers about the quality of care provided by nursing homes available at www.medicare.gov. THE IOWA FOUNDATION FOR MEDICAL CARE HAS SUPPORTED EFFORTS TO IMPROVE THE PROVISION OF CARE TO DEPRESSED NURSING HOME RESIDENTS AND I HAVE BEEN OFFERING INPUT SAY MORE ABOUT THIS
