Reporting Back on question FOG Presentations: Two Fall of Ground Incidents were presented. On 05 December 2017 at approximately 14H30, Miss Lethebe was fatally injured at Tshepong Mine following a fall of ground incident that occurred in the 71 98 P1 Wide Raise Waste Cut. On Saturday, 18 November 2017 at approximately 09h10, Mr Khalikana was fatally injured at Masimong Mine following a fall of ground incident that occurred in the 1750 SW W6 Panel E1. 6
Reporting Back on question General Time delayed failure of tendon support units, due to rust and corrosion was discussed. It was requested that a presentation to be done on Galvanised grout tendons, as a possible solution where ground water is present as well in areas such as Return Airways where high humidity is present. Consideration must be given to limit panel face lengths to a maximum of 30m ensure proper cleaning, timeous installation of support and daily blasts. Tri-partite meeting – every second Tuesday of the month. 7
