South west London Transforming Care Partnership - Who are we? Tim Bennett – Transforming Primary Care Clare Thomas, Engagement

Why are we here? We want: To talk to you about what is happening in your local community to improve care for people with learning disabilities Get your advice on how to speak to people with learning disabilities about this work

What is the plan to improve care? These people wrote plan called “Building the right support”.

The plan says, people with learning disabilities must be supported to: Live where they choose Get support with their health in the community Get health treatment near their home when they need it Only stay in hospital for as long as they need to.

What is a Transforming Care Partnership? The Transforming Care Partnership is made up of people who are in charge of local services. These are Clinical Commissioning Groups Local councils NHS England Carers A Transforming Care Partnership has been set up in each area of London

Transforming Care Partnership in south west London What areas do we cover? Croydon Kingston Merton Richmond Sutton Wandsworth

Why are we working on Transforming Care Partnership? Lots of people in special hospitals are far away from their friends and family And sometimes people stay longer in these special hospitals than they should do

Our aims We want to support people in their homes and reduce the number of special hospitals We want people to get support in the community, helping families to stay together in the areas they know We want people to get the right treatment closer to where they live

We know… 51 people from south west London are in special hospitals. These hospitals are not in south west London 6 of these people are under the age of 18

People must be at the centre of our plans We need to work together with people who have Learning Disabilities and/or autism so that services are better in the future

How we are working together 1. Wider reference group – we have formed a group of local organisations and carers to tell us their experiences 2. We would also like to talk to people who use services – and we need your help.

What we need your help with… 1. Who to speak to? People in special hospitals People who use community services Carers Advocates 2. Where should we go? Day centres Drop ins Supported living 3. How? On the phone In person Working with peer advocates Who else should we speak to? Would it be a good idea to speak to Doctors/Nurses? Support staff?
