Eligibility Not automatic! SVM must update DEERS information Contact local ID card facility Locations on DEERS website
Requirements Involuntarily separated W/ qualifying SPD code Reservist on AD > 30 days Invol. Separations retained on AD for operations Agreed to extend, but separated on sole survivorship discharge Must agree to SELRES IF no qualifying SPD code-work w/ reserve unit

Community Based Warrior Transition Units (CBWTUS) Soldiers live at home, recuperate, while on AD Provides high quality of health Can perform duties close to home and family Evaluated by PEB if not found physically fit at the end of recuperation period. Retainablity Disability eligibility
Retirees Begins the day you retire Submit request 90 days prior Dependents remain eligible To continue coverage: Must enroll by retirement date Will default to space only coverage
