On-Call Issues for Geriatric Fellows UH Department of Geriatrics You’re Being Paged… On-Call Issues for Geriatric Fellows UH Department of Geriatrics

You all will be starting call soon…. What has been your call experience in the past? What is the worst part about call? What are the hardest things to deal with?

Care of the nursing home patient is different than general Internal medicine or Family medicine… You manage a lot more over the phone Patients are often more complicated On the bright side, there is a nurse at the other end of the phone.

Let’s run through some scenarios… Two fellows per scenario: One is the fellow One is the nurse calling, this person will also be the family member if applicable We will watch and offer constructive suggestions and make comments Relax! This is just practice!

The “nurse” fellow will be given a sheet with additional information the physician fellow should elicit with questions Nurses: Do not volunteer information. Let the “fellow” elicit the information from you. “Fellow”: If you are stumped, you can ask your colleagues here to help you! Just say, “I want to call my attending and call you back.”

Doctor, the U/A has 2+ leukocytes and 2+ nitrates…

Doctor, Mr. _____doesn’t look good…

Doctor, Mrs. _____has a rash…

Mr. ___is refusing his medications!

Mrs. ___’s family wants an update…

Mrs. ____is wheezing…

Mr. ____has diarrhea…

I need an order for cough syrup for Mr. ____, Doctor.

Mr. ___’s fever won’t go away and we gave tylenol 2 hours ago…

Mrs. ______’s G-tube came out and I think she needs to go to the hospital right now!

Any other scenarios? How was it? We’ll meet monthly to discuss and dissect particularly difficult “on-call” issues We’re here to support you! Thanks for your participation!
