ENCOURAGE Participants TO COMPLETE THE HSA APPLICATION AS QUICKLY AS THEY CAN ONCE THEY LAUNCH IT, TO AVOID SECURE SITE TIMEOUT THAT WILL STOP THE APPLICATION IN THE MIDDLE (time for timeout depends on internet service provider, so we can’t give a “Do it in XX minutes or else” timeframe) Click the “Finish Opening Your HSA” button to launch the HSA application

Confirm Your Information Most, if not all, of this will carryover from what the entered on the WW site.

Select Paper or E-mail Statements $ 0.75 deducted from HSA balance each quarter if you select Paper statements

Beneficiaries can be added (or changed) later, as well Add Beneficiaries Beneficiaries can be added (or changed) later, as well

Post-tax (personal) contributions Select an Optional box if you expect to make post-tax (personal) contributions directly to BNY Mellon (not via payroll) This will not actually affect the setup of the account in any way, Participants can usually be encouraged to just ignore this page and leave the Optional boxes unchecked.

Confirm your information Almost done… be sure to click Finish and Submit

Confirmation Once the application is complete (or on 12/11, whatever comes later) Card production/mailing will begin. If they also signed up for the LFSA, card production starts on 12/11, after close of the enrollment period, even if the HSA application is not completed.
