The Primary Care Almanac Just as a reminder, you can access your Almanac from the tools menu in CPRS (It may be listed differently than you see here) or by logging into VSSC at VSSC.med.va.gov. It is crucial that that staff be knowledgeable on how to access this information. Just as important as it is to Identify patients with uncontrolled medical issues at higher risk for adverse outcomes ow to use this information to improve their care with additional resources. In addition, we need to work to restructure the care we give from being reactive and defensive system to the model that is proactive in the care we deliver to patients. PACTs can bring care coordination to the forefront by promoting discussions of this during huddles and care planning sessions. There is an enormous amount of information available in the Primary Care Almanac, and a complete discussion here is not practical.
