What do we mean by transition? Transfer of young people out of Child & Young People Mental Health Services to other services or to discharge CAMHS transition point is at 18 years
Why are we concerned about transition? Can be anxiety-provoking for young people & families Poor transition can lead to: reduced engagement with adult services emergency admissions and crises unnecessary repeat assessments gaps in care TRACK study (Singh, 2008) – only 4% of young people received an ‘ideal transition’ 25-50% of under 25s disengaged from mental health services (Birchwood, 2010)
What does good transition look like? Person-centred, involve the young person Involve families and carers when appropriate Joint working between child and adult services Uninterrupted Involve young people and their carers in service design, delivery and evaluation Named and contactable transition key worker
CAMHS transition process Transition planning starts at 17years, 3months Discussed in transition panel, transition passport Consider: background, other agencies involved, current mental health needs, risk, resilience, input receiving from CAMHS, engagement in services, goals Complete transition plan at least 6 months prior to transitioning Transition meeting in CAMHS and in adult services Named and contactable transition key worker – lead professional/care co-ordinator Transition plan shared with GP and other relevant services
Transition passport Completed with young person Includes: Strengths and difficulties Hopes and aspirations for the future Triggers Emergency/crisis plan What support I need from my new service Preferences about my care History of care interventions Contact details for local services
Transition panel Discussed at 17 years, 3 months Information provided by lead professional/care co-ordinator Panel attended by: CAMHS Adult Secondary Care Mental Health Services Improving Access to Psychological Therapies (IAPT) Adult Learning Disability services Early Intervention in Psychosis service Multi-agency case discussion and planning Fed-back to lead professional/care co-ordinator, young people & families Liaison with local organisations e.g. Converge, Mind
