Event March 2016 Aims was to explore the development of future Rehabilitation and Recovery services in York & Selby. Providing an opportunity to consider a new vision and model of rehabilitation services and seamless care pathways in York & Selby, encompassing a wide spectrum of partner organisations that support individuals in their community Over 3 days 80 participants 18 of which service users and carers Many representations from voluntary and non nhs agencies

There are five key desired service outcomes Reduce reliance on Rehabilitation inpatient facilities Ensure recovery pathways are as seamless as possible Improved partnership working to enable continued recovery in the community A more efficient, evidence based, high quality service To support people in Out of Area treatment placements to return to the York & Selby Locality review of the configuration of the Rehabilitation and Recovery service to ensure there are effective pathways across a network of services and wide spectrum of support, determined by local need. Some components of an effective Rehabilitation care pathway may be provided by independent and third sector organisations as well those provided by the Trust

The final vision following voting 18 SU’s and carers, LA, Vol services, Students, inpatients and community staff, of all discliplines.

Mission Statement Creating a mental health service which gives people hope for positive change and recognises the importance of listening to the individual. Building pathways to recovery which allow individuals to go at their own pace, with the freedom to make informed choices about their care. Developing and maintaining partnerships with the community and other organisations to keep lines of communication clear and avoid leaving people with out support. Taking a holistic approach to the strengths and vulnerabilities of each individual, combining physical and mental health with life skills and self care. 1 Hope for positive change & listen 2 own pace & make informed choices 3 partnerships with the community are essential / lines of communication clear / avoid leaving people with out support. 4. strengths and vulnerabilities

Whole Concept Buttercup and flower network – representing the community working together for recovery. Tree at centre – represents knowledge, skills and experience, shared values, heart of network. Topped by the owl of wisdom and diversity.

Hive The Door Queen Bee Snails Bees Buttercups and flowers An obvious door at the centre of the tree represents clear, visible access. A straight forward route for urgent access. Queen Bee A person seeking mental health support. They are at the heart of the network. All the worker bees are working for the queen bee. Snails The service is paced to the individual and can go at the person’s pace. Hive The secure base for the queen bee – whether that is home or a resource in the community. The worker bees are working to make that place secure and comfortable. Bees Are representative of all service workers with different connected roles. (Health workers, educators, researchers, council staff, volunteers, friends, carers, family.) Buttercups and flowers Physical location of service provision. Health facilities Education facilities. Third sector organisations. Community organisations. Connected by a strong root network. Other flowers and overflowing roots A wider range of resources and providers. The worker bees are constantly looking beyond their network for new connections. Swing A willingness to try things and to take risks. Nest Somewhere to feel safe /sanctuary when needed. Time out when needed Snail – at the own pace.

Agreed next steps The group have agreed to develop a focused consultation group, reporting to the project group and supporting other community developments. This became the Rehabilitation and Recovery Steering group

First job was to interpret the model into action

Team to engage with service users out of area and locality, to fully understand what the needs are. Scoping the possibility of a Crisis café Enhance community serivces MH Steering Group – getting past barriers in mental health and housing support.

Repatriate service users to York Links to the Whole Concept - The Hive and Nest Each service user has a recovery plan Review and transition panel Appointed an advanced practitioner support those placed away from York to return when appropriate. who’s role it will be to connect with the people in out of area placements and repatriate them to the local area

The International Whole Person, Whole Life-Whole System Recovery Symposium Links to the Whole Concept - Snails, Bees, Buttercups and Flowers, Other flowers and overflowing roots, Swing Developing a shared vision for Mental Health in York Involves working on the real needs and interests of people Challenging familiar, outdated concepts and ideas

Enhancing Community Rehabilitation Treatment. Links to the Whole Concept - The Door, Queen Bee, Hive, Swing and Nest The long term vision is a cultural move from traditional bed base / admissions to caring communities, supported by community facing services. Pilot team in place. It will provide intervention and treatment for the gap in provision between an in-patient units and supported housing accommodation or independent living that can be difficult to manage for some of service users.

The Haven @ Clarence Street Links to the Whole Concept - The Door, Queen Bee, Hive, Swing and Nest Opened on the 31st October 2017 From 29th Jan 2018 times; 6 – 11 pm An out of hours service for any adult in mental distress in York and Selby A welcoming, safe, supportive, comfortable and non-clinical environment that promotes independence opportunities for recovery Designed by service users for service users

What next? The group had achieved what is was able to do in its current form In line with recovery aims, ie connectively and inclusion, the group will move outwards and integrate with the whole systems approach
