PROGRAMME Advance Care Planning and Communication Skills Facilitator 2 day Training Course Dates: Thursday 15th & Friday 16th March 2018 and Thursday 7th & Friday 8th June 2018 Are you required to educate and support staff in your role? Do you have clinical or practical experience of supporting frontline staff in Advance Care Planning? Are you interested in becoming an Advance Care Planning Champion in your own workplace? Do you want to have the skills and knowledge to train and support others in planning for future care? Free places for this two day Train the Facilitator Course are available for health and social care staff thanks to funding from Health Education England. Advance Care Planning and Communication Skills Train the Trainer Facilitator Training Course This 2 day Train the Trainer Facilitator course offers the opportunity to develop the knowledge, skills and understanding to deliver a 1 day Advance Care Planning (ACP) education day to staff in your organisation The course will prepare facilitators to become Advance Care Planning Champion, cascading learning, mentoring staff and promoting ACP within their organisation Facilitators are expected to attending quarterly ACP Facilitator/Champion forum meetings You will be supported and mentored with your personal development in this role Have the opportunity to attend the 1 day Advance Care Planning education day prior to the 2 day Train the Trainer course Course Applicants should have….. The support of their manager clinical and/or practical experience within current role relevant to staff who will be trained and supported with ACP Knowledge of ACP, mental capacity Act, best interest decision making, legal and ethical implications Education/training skills Communication skills training
