What is it? Military Separation Physical/Disability Exam with the VA Establishes current medical conditions, medical history, and future support for ongoing medical needs. Scheduled at a military hospital or clinic or at a VA facility Guidance outlined in DoD Instruction 6040.46 The SHPE for the DoD Separation Health Assessment (SHA) Program DTM14-006:Separation History and Physical Examination MOA Implementation of Cooperative Separation Process/Examinations at Discharge Sites Medical and Dental Records STR Service Treatment Record
Why is this important? Gives a complete medical history of the TSM and evaluates the member’s disability claim Results are accessible by both DoD and the VA which allows claims to be filed at a later date. Required for all active duty TSMs prior to separation Required for all Reserve Component that have served 180 days on active duty or 30 days in contingency operations Record must be maintained by MTF or medical clinic for future VA disability benefits.
When should I complete the SHPE? Schedule your exam no later than 90 days before your separation date. If on extended terminal leave, military hospital or clinic is best Complete DD Form 2807-1 “Report of Medical History” If filing a VA claim, no later than 90 days Call the VA or military clinic/hospital for an appointment DD form 2507-1 before the exam-used to see if further treatment or evaluations are needed
