Chester County Immunization Coalition Adult Vaccine Project 2017 Goal: To increase adult vaccination rates in Chester County. Phase One: Recruit vaccinating partners (pharmacies, convenience and travel medicine clinics) to enter their vaccine services in HealthMap Vaccine Finder - a free, online database where users search for locations that offer vaccine services. www.vaccinefinder.org Phase Two: Educate physicians in Chester County about recommended vaccines for adults and HealthMap Vaccine Finder website as a place to refer patients for vaccines not provided in their medical home. Phase Three: Educate adults in Chester County about recommended vaccines and where to find them.
