Forms Needed to Participate in Athletics OHSAA Physical Form Emergency Medical Card Ohio Concussion Form Student-Parent Contract *High school students are required to undergo IMPACT testing with athletic trainers at the beginning of the season*
Participation Fees $150 at the high school $100 at the middle school Payment plan dates March 15 March 29 April 12 April 26 *Any missed payment will result in the athlete not being able to participate in future scrimmages and/or games until payments are current.*
Attendance Must be in school half a day High School: 7:20am-10:45am or 10:45am-2:10pm Middle School: 7:45am-11:45am or 11:45am-2:40pm Exceptions can be made if approved ahead of time by building principal (High School=Gene Lolli, Middle School=Deb Hauberg) Must provide doctor’s note if missing more than half day for medical reason
Eligibility Must pass 5 one credit classes Must maintain a 2.0 GPA Every student may use a one time exemption if their GPA falls between 1.5 and 1.9 This exemption may be used one time only in high school and one time only in middle school Students must live in Fairborn school district May attend Baker MS, Fairborn HS, Greene County Career Center, Dayton STEM, Homeschooled, other charter/community schools as approved by Fairborn Schools
Transfer Students *Any student who transfers during or after their freshmen year must fill out OHSAA transfer paperwork with Mr. Harding in the athletic office before they can Contact Mr. Harding with any questions.play in a scrimmage or game*
Drugs, Tobacco & Alcohol Guidelines 1st Offense Must sit out half the season of current sport or upcoming sport Must attend 6 substance abuse counseling sessions Must show proof of a passed drug test within a week of returning to play Cannot have any office/discipline referrals during 50% suspension 2nd Offense Must sit out from athletics for 365 days 3rd Offense Banned from athletics for remainder of career playing sports at Fairborn HS
Communicating with Coaches Communication should always be player to coach first Parent can contact coach to set up a meeting. Strongly encouraged that the student-athlete be present during the meeting If the coach cannot be reached in a reasonable amount of time, contact the Athletic Director. Communicating with Coaches *Never attempt to confront or contact a coach before , during or after a game or practice* 24 hour rule Email/call/text the coach to set up the appointment
