Human Participants [Form (4)] The Institutional Review Board will complete this section. It must have a school administrator, educator, and medical or mental health professional. The school nurse is not able to sign. It can be the school psychologist, medical doctor, physician’s assistant (See form for more details) Summarize your research in three to five points.
